While the self defense move is demonstrated in slow motion, you would actually move your lead hand and your elbow at almost the same time. If you're moving with speed and with both arms at the same time, it's literally impossible for him to react in time to "punch" you as a result of getting grabbed by the back of the head unless he's already in the process of punching, but again...in close quarters, you're usually too close to punch anyway.
